789JAM IDs
To receive a 789 JAM ID, you MUST show your school ID (to verify your grade level). Please complete the application below, and bring your student ID to pick up your 789JAM ID at the next dance you attend.
These IDs will contain your picture and first name (NO LAST NAME), an ID number and an expiration date. They will be used to verify your eligibility to attend the dances, and to look up parent contact info in the event of an emergency. Finally, with your 789 JAM ID card, you will also receive a $2 discount off the $10 admission price.
This form will close at midnight the day before a scheduled Dance Party. Students will have an opportunity to make a 789JAM ID inside the dance.
